## Question Wording Rules
- One idea per question — never double-barrelled ("Is the product fast and reliable?")
- Avoid leading questions ("Don't you agree that...?")
- Use simple, common vocabulary — aim for grade-8 reading level
- Avoid negatives in response options ("not dissatisfied")
- Define ambiguous terms (what is "recently"? "often"?)

## Scale Selection
- Likert (agree/disagree): use 5 or 7 points, with a midpoint
- Frequency: "Never / Rarely / Sometimes / Often / Always"
- Satisfaction: 1–10 NPS for benchmarking
- Always label all points on the scale, not just endpoints

## Survey Structure
1. Brief purpose statement (1 sentence)
2. Completion time estimate
3. Easy questions first to build engagement
4. Sensitive or demographic questions last
5. Open-ended questions at the end, never in the middle
6. Thank-you page with next steps

## Bias to Avoid
- Social desirability: anonymise when asking about sensitive behaviour
- Acquiescence: mix agree/disagree directions
- Order effects: randomise item order for attitude batteries

## Pilot Testing
- Test with 5–10 people from the target population before launch
- Ask them to think aloud while answering
- Record confusion, hesitations, and abandoned responses
